I understand that participation in Jiu Jitsu, grappling, wrestling, self-defense training, fitness training, martial arts instruction, sparring, competitions, seminars, open mats, and related activities involves inherent risks. These risks include, but are not limited to: Cuts, bruises, and abrasions Sprains and strains Joint injuries Broken bones Concussions and head injuries Neck and spinal injuries Serious bodily injury Temporary or permanent disability Death I voluntarily choose to participate in these activities with full knowledge and acceptance of these risks. 2. ASSUMPTION OF RISK I knowingly and voluntarily assume all risks associated with participation in activities offered by Two Wolves Jiu Jitsu, whether such risks are known or unknown, anticipated or unanticipated. I acknowledge that injuries may result from: My own actions or omissions The actions of other participants The condition of the facility or equipment The instruction or supervision provided Accidental contact during training 3. RELEASE OF LIABILITY In consideration for being permitted to participate in activities at Two Wolves Jiu Jitsu, I hereby release, waive, discharge, and covenant not to sue: Two Wolves Jiu Jitsu Its owners, coaches, instructors, employees, volunteers, contractors, and representatives from any and all claims, demands, damages, losses, liabilities, costs, or expenses arising out of or related to any injury, illness, property damage, disability, or death resulting from participation in academy activities, except where prohibited by applicable law. 4. MEDICAL AUTHORIZATION I certify that I am physically capable of participating in martial arts training. I agree to immediately notify instructors of any medical condition, injury, or limitation that may affect my participation. In the event of an emergency, I authorize Two Wolves Jiu Jitsu personnel to obtain medical treatment on my behalf. I understand that I am solely responsible for all medical expenses incurred. 5. HEALTH AND HYGIENE POLICY I agree to: Maintain proper personal hygiene Wear clean training attire Keep fingernails and toenails trimmed Report any contagious condition, infection, rash, or skin disease Refrain from training when ill or contagious I understand that failure to follow these requirements may result in suspension from training until the issue is resolved. 6. RULES AND CONDUCT I agree to follow all academy rules and instructor directions. I understand that unsafe, reckless, threatening, abusive, or inappropriate behavior may result in immediate removal from class and possible termination of membership without refund. 7. PHOTO, VIDEO, AND MEDIA RELEASE I grant Two Wolves Jiu Jitsu permission to photograph, record, and use my image, likeness, voice, and appearance in promotional materials, social media, websites, advertisements, and academy publications without compensation. If I do not wish to be photographed or recorded, I will notify academy management in writing. 8. MINOR PARTICIPANTS As the parent or legal guardian of the minor participant listed above, I consent to the minor's participation in academy activities. I agree to all terms of this Agreement on behalf of myself and the minor participant and accept responsibility for the minor's conduct and participation. 9. SEVERABILITY If any provision of this Agreement is found unenforceable, the remaining provisions shall remain in full force and effect. 10. GOVERNING LAW This Agreement shall be governed by and interpreted under the laws of the State of Colorado. 11. ACKNOWLEDGMENT I have carefully read this Agreement and fully understand its contents. I understand that I am giving up substantial legal rights, including the right to sue. I sign this Agreement freely and voluntarily without inducement.
